Immigration Physicals

Instructions for Immigration Physicals:
- The fee for the physical exam does NOT include any vaccinations provided.
- The Physical exam fee includes all required and other health screening labs.
- All medical records and vaccination documentation should be brought on the first office visit.
- All physical examinations are valid up to 1 year from the date of exam.
- We will need at least 5 working days to process any exam results given by the laboratory.
- The completed I-693 form will be provided to you in a sealed envelope.
What you need to bring with you
- Picture identification such as a passport or other Federally approved documentation (state identification such as a driver’s license and school ID is not acceptable).
- Your medical records for any chronic or serious medical problem (translated in English).
- Your immunization records, even childhood records, if available (translated in English).
- Alien Registration number if you have one (starts with an “A”)
- Fill the Medical History form before coming for the appointment.
- The I-693 form if you have one, or else we will provide it.
