Immigration Physicals

Instructions for Immigration Physicals:

• The fee for the physical exam does NOT include any vaccinations provided.
• The Physical exam fee includes all required and other health screening labs.
• All medical records and vaccination documentation should be brought on the first office visit.
• All physical examinations are valid up to 1 year from the date of exam.
• We will need at least 5 working days to process any exam results given by the laboratory.
• The completed I-693 form will be provided to you in a sealed envelope.

What you need to bring with you

• Picture identification such as a passport or other Federally approved documentation (state identification such as a driver’s license and school ID is not acceptable).
• Your medical records for any chronic or serious medical problem (translated in English).
• Your immunization records, even childhood records, if available (translated in English).
• Alien Registration number if you have one (starts with an “A”)
• Fill the Medical History form before coming for the appointment.
• The I-693 form if you have one, or else we will provide it.
