Overview
The Immigration Medical Examination (Form I-693) is required by U.S. Citizenship and Immigration Services for most applicants seeking lawful permanent resident status. The exam must be performed by a USCIS-designated Civil Surgeon.

Syphilis and gonorrhea testing for applicants age 18 and older, as required by USCIS.

Review of your immunization records and on-site administration of any required vaccines.
Your completed and signed Form I-693, sealed in the USCIS-required envelope and ready to submit.

Vaccination requirements
Bring all prior vaccination records — documented vaccines do not need to be repeated. Missing vaccines can be administered during your visit.

What to bring
Form N-648
The N-648 waives the English and/or civics requirements for naturalization applicants with a qualifying physical, developmental, or mental impairment expected to last 12 months or longer.
The N-648 must be signed within 6 months of submission to USCIS. Please call ahead so we can review prior records before your visit.
