1. Overview

This policy explains what information we collect when you visit simplycare.net, how we use it, and who we share it with. SimplyCare is operated by Ashok Mehta MD PC, which is affiliated with Privia Health. This policy covers this website only. How we handle your medical records is described in our Notice of Privacy Practices (see section 13).

2. Information we collect

Information you give us. When you fill out a form on this website we collect what you enter, such as your name, phone number, email address, the service and appointment times you select, and anything you type into a message or notes box — including any health information you choose to include. Some forms let you upload documents.

Information collected automatically. When you visit, we and our providers may collect your IP address, device and browser type, pages you visit, the page that referred you, ad click identifiers (such as the Google click ID, “gclid”) and campaign (UTM) parameters, and information stored in cookies or similar technologies.

3. Website forms are not for detailed medical information

Our website forms, including message boxes, are not a secure channel for medical records and are not intended for detailed health information. Please do not submit sensitive health details such as diagnoses, medications, test results, or insurance ID numbers. Share those by phone, in person, or through the patient portal. Information you send through this website is transmitted over an encrypted HTTPS connection, but the website forms are not part of your medical record.

4. How we use information

To respond to your request, contact you, and schedule appointments.
To send confirmations or follow-ups you asked for (text messages only with your consent).
To operate, maintain, and secure the website, including preventing spam and abuse.
To understand how the website is used and measure how well our advertising works.

5. How we share information

We share information only as described here:

Our staff, through internal team messaging and email notifications so the right person can call you back.
Our lead-tracking database, which records your name, phone, email, the page you came from, your time preference, the ad click identifier, and the status of your request.
Our email delivery provider, which sends staff notifications about new requests.
Our SMS provider (Twilio), only to deliver text messages you opted in to receive.
Hosting, database, and security providers that run this website and store form submissions, including a spam-prevention check on some forms.
Google. We use Google Analytics and Google Ads tags, which set cookies and record page visits and form-submission events so we can measure website use and ad performance. If you arrived from a Google ad, we may later send Google the ad click identifier (gclid) and the time of a conversion (for example, that a visit took place) to measure ad results. We do not send your name, phone number, email, or message content to Google for this purpose.
As required by law, or to protect the rights and safety of our patients, staff, and others.

We do not sell your personal information and do not share it for third-party marketing.

6. Cookies, tracking, and how to opt out

You can block or delete cookies in your browser settings. You can manage Google ad personalization at adssettings.google.com and opt out of Google Analytics with the Google Analytics opt-out add-on. Our website does not currently respond to browser “Do Not Track” signals. Some pages include embedded Google Maps or Google Drive video content, which may set Google cookies.

7. SMS / text messaging

If you check the SMS consent box on one of our forms, or sign the SMS consent section on our in-clinic intake form, you agree to receive text messages from SimplyCare about your request, appointment reminders, and care updates at the phone number you provide. Message frequency varies. Msg & data rates may apply. Reply STOP to opt out at any time or HELP for help. See our SMS Program Terms for full details.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All other categories of data are subject to the rest of this Privacy Policy. Information shared with subcontractors (e.g., our SMS delivery provider) is used solely to deliver messages you have opted in to receive.

8. Security

We use reasonable administrative, technical, and physical safeguards to protect information submitted through this website. No method of transmission or storage is 100% secure, so we cannot guarantee absolute security.

9. How long we keep information

We keep website submissions only as long as needed to respond to you and schedule care, and as needed for legitimate business and legal record-keeping.

10. Children under 13

This website is not directed to children under 13, and we do not knowingly collect their personal information online. If you believe a child has submitted information, please contact us and we will delete it.

11. Links to other websites

Our website links to other sites, such as the patient portal, Privia Health, and partner sites like SimplySkin, The DNA Diet Club, and DeepDive MD, and includes embedded videos and maps. Those sites and services have their own privacy policies, which apply when you use them.

12. Your choices and rights

You can ask us to access, correct, or delete information you submitted through this website.
You can opt out of text messages at any time by replying STOP.
You can manage cookies as described in section 6.

13. Your medical records (HIPAA)

How we use and protect your medical information as a patient is described in our Notice of Privacy Practices.

14. Changes to this policy

We may update this policy. Changes will be posted on this page with a new effective date.
