What the exam includes

Medical history

Your medical, medication, and vaccination history, as required on Form I-693.

Physical examination

A head-to-toe physical by Dr. Mehta, the civil surgeon, following CDC Technical Instructions.

Vaccination review

We review your records against current USCIS requirements; any missing vaccines can be given in the office.

TB and syphilis blood tests

One blood draw covers the TB blood test (IGRA) and the syphilis blood test, as required by USCIS (syphilis testing currently applies to applicants ages 18–44).

Sealed Form I-693

Signed civil surgeon form in a sealed envelope — pick it up once your labs return.

Exam and TB blood draw done in a single appointment. Lab results come back in 3–5 business days — you return to pick up your sealed I-693 envelope.

Do not open the sealed envelope — USCIS requires it intact at submission.

Flat rate from $400 for the exam, civil surgeon signature, and sealed envelope. The TB blood test and any vaccines you still need are billed separately — call and we'll give you your exact total.
